Most women get one prenatal vitamin, generic prenatal nutrition advice, and a "see you at your next appointment." You deserve to understand what's actually happening in your body, and the data to back it up.
Why Trimester-Specific Matters
Here's what most people don't realize: a lab value that looks "abnormal" in the third trimester might be a completely normal pregnancy adaptation, and a value that looks "normal" by standard ranges might actually signal a developing issue, because pregnancy changes what normal even means.
Blood volume expands. Hormones shift dramatically. Iron and mineral demands climb as your baby grows. Testing the same panel at the same intervals throughout pregnancy misses all of it.
That's why I don't run one generic panel and call it done. Whether you're preparing to conceive or already expecting, I test based on where you are: first trimester pregnancy, second trimester pregnancy, third trimester pregnancy, or postpartum recovery. That's where the answers actually are.
First Trimester
The foundation.
This is where we establish your baseline, ideally within the first few weeks of a confirmed pregnancy, before six weeks when possible. Early testing means we catch what's happening before the bigger physiological shifts of pregnancy take over, so we know what's truly you versus what's the pregnancy.
What we're assessing: thyroid function, since shifts here affect your energy, your mood, and your baby's development; early blood sugar regulation, which is often behind first-trimester nausea and fatigue; iron status; key nutrient levels that support a healthy pregnancy; mineral status; and your core reproductive hormones.
If anything is suboptimal here, we have time to act. Most miscarriage risk tied to hormonal factors plays out in this window, which is exactly why we don't wait.
Second Trimester
Watching the shift.
This is the trimester where insulin resistance starts to build and blood volume expansion accelerates, both of which can quietly set the stage for gestational diabetes if they go unmonitored. We're not waiting for your glucose screening to tell us something's off. We're watching it happen in real time.
What we're assessing: continued metabolic and blood sugar regulation, since this is where gestational diabetes risk really builds; iron trends; key nutrient status; and your overall wellness markers, reassessed against second-trimester norms rather than standard, non-pregnancy ranges.
This is also where a lot of lab values start to look "off" if you're checking them against non-pregnancy ranges. I'll walk you through exactly what's expected here so you're not left googling your own results at 11pm.
Third Trimester
Preparing your body for labor and recovery.
Iron depletion accelerates here to support your baby and placenta. Mineral demands peak. This is the trimester where getting ahead of deficiencies actually matters, because there's a real window: testing at 33–35 weeks leaves enough time to correct course before delivery. Testing at 38 weeks doesn't.
What we're assessing: iron status, since this is a critical window (weeks 33 to 35) to catch and correct depletion before delivery; mineral status, particularly the minerals that affect cramping, sleep, and labor prep; a thyroid recheck; and continued metabolic monitoring.

Postpartum
The window everyone forgets to check.
Postpartum is the most dramatic hormonal shift your body will ever go through, bigger than puberty, bigger than pregnancy itself. Estrogen and progesterone drop fast, your thyroid has to recalibrate from pregnancy levels back down to your new normal. Postpartum thyroiditis is common, and most providers simply aren't watching for it. That's why "this is just what postpartum feels like" gets said so often. Frequently, it isn't.
We start testing around 6–8 weeks postpartum. That's enough time for your body to begin settling, early enough to catch what's developing before it becomes entrenched. And in those first three months, we're not rushing to intervene. Your body is doing the work of finding its new baseline, and gentle nutritional support goes further than aggressive correction during postpartum recovery.
What we're assessing: a full thyroid panel, since postpartum thyroiditis often runs in two phases, a wired, anxious, can't-sleep phase first, followed months later by an exhausted, foggy, can't-lose-the-weight phase, and knowing which phase you're in changes everything about how we support you; iron status, since depletion after birth is close to universal, regardless of birth type, and is rarely addressed proactively; blood sugar and insulin regulation, as your metabolism resets and sleep fragmentation can throw glucose around more than people expect; key nutrient levels; and mineral status.
